Showed up with skin changes and poor visual acuity in both eyes. In ophthalmologic exam the patient had visual acuity only to “vultos” in both eyes. A biomicroscopy noted in the right eye corneal infiltrate higher with the presence of precipitates ceráticos pigmented, "flare" and pearls and absence of pupil. In the left eye corneal infiltrate higher with the presence of precipitates ceráticos pigmented, "flare" and pearls and irianos nodules, absence of pupil. Was the diagnosis of ceratouveite Hansênica, and patient transferred to the Center Marcelo Cândia for treatment of leprosy, then undergo surgical correction of cataract bilaterally as well as iridectomia for biopsy, occurring procedure without complications. The final diagnosis confirmed by baciloscopy of the lymph and histopathology of iris. Currently the patient is in treatment of the changes produced by leprosy, presenting a visual acuity of 20/40 in right left eye and 20/30 in left eye, with correction. Este trabalho relata um caso de comprometimento ocular pelo Mycobacterium leprae, bactéria causadora da Hanseníase, em um paciente procedente da cidade de Ananindeua, que se submeteu à consulta oftalmológica com o objetivo de avaliação da acuidade visual e retiana. Apresentava-se com alterações dermatológicas e baixa acuidade visual em ambos os olhos. Ao exame oftalmológico o paciente apresentava acuidade visual (AV) somente de vultos em ambos os olhos. À biomicroscopia observava-se no olho direito (OD) infiltrado corneano superior com presença de precipitados ceráticos pigmentados, “flare”(+), células inflamatórias (+) e pérolas hansênicas irianas e ausência de pupila. No olho esquerdo (OE) infiltrado corneano superior com presença de precipitados ceráticos pigmentados, “flare”(+) e pérolas hansênicas irianas , nódulos irianos e ausência de pupila. Foi estabelecido o diagnóstico de Ceratouveite Hansênica, sendo o paciente encaminhado para o Centro Marcelo Cândia para tratamento da Hanseníase, a seguir submetido a correção cirúrgica da catarata bilateralmente, além de iridectomia para biópsia, procedimento ocorrendo sem intercorrências. O diagnóstico definitivo foi confirmado, através de baciloscopia da linfa e histopatológico da iris.
