Its capital, Belém, has one of the highest incidence rate of the world. In addition to the Helicobacter pylori infection and factors related to lifestyle, genetic susceptibility has been linked to individual variation in risk of developing this type of cancer. Despite advances in surgical techniques and adjuvant therapy, survival rates in five years in patients whose tumors are resected, remains extremely poor because the disease is usually asymptomatic in its early stages, preventing early diagnosis. Genetic polymorphisms are associated with the development of cancer when combined with environmental factors and can be considered biomarkers for studies involving the risk of developing the disease, allowing for screening and early diagnosis. The present study investigated the polymorphism PIN3 in the gene TP53, which consists of a INS/DEL 16 bp in intron 3, with patients with gastric cancer in the University Hospital Joao de Barros Barreto and with a control group from Belem’s mixed population between july 1st, 2009 and march 1st, 2011. We collected blood samples from 99 patients with gastric cancer and 225 healthy controls. DNA was extracted by the method described by Sambrook et al. (1989). We used the technique of polymerase chain reaction for genotyping the samples. The results indicate an association of gastric cancer among males (67.7%) and age (mean 59.3 ± 1.2 years), predominance of Lauren’s diffuse-type and advanced stage disease (70.7% had TNM III/IV). We found a significant association of susceptibility to gastric cancer for genotypes INS/DEL (p = 0.007, OR = 3.990) and INS/INS + INS/DEL (p = 0.007, OR = 3.554) as well as average acceleration of approximately 15% in time of onset of disease in patients with INS genotype (p = 0.002, OR = 1.15). Genotype DEL/DEL presented related to protecting the development of gastric tumor (p = 0.020, OR = 0.343). These results suggest that PIN3 may be associated with the development of gastric cancer, as well as contribute to the formation of a panel of markers for its diagnosis. O câncer de estômago é a segunda principal causa de morte por câncer em ambos os sexos em todo o mundo e é considerado um problema de saúde pública no Estado do Pará devido à sua alta incidência e mortalidade. Belém apresenta uma das maiores taxas de incidência de câncer gástrico do mundo. Além da infecção pelo Helicobacter pylori e fatores relacionados ao estilo de vida, a susceptibilidade genética tem sido associada à variação individual no risco do desenvolvimento deste tipo de câncer. Apesar do avanço nas técnicas cirúrgicas e na terapia adjuvante, as taxas de sobrevida em cinco anos de pacientes cujos tumores são ressecados permanecem extremamente insatisfatórias, pois a doença é, geralmente, assintomática no seu estágio inicial, o que impede o diagnóstico precoce. Os polimorfismos genéticos são associados ao aparecimento de câncer quando combinados com fatores ambientais e têm sido considerados biomarcadores para estudos envolvendo o risco do desenvolvimento da doença, permitindo rastreamento e diagnóstico precoce. O presente trabalho investigou o polimorfismo PIN3 no gene TP53, que consiste em INS/DEL em 16 pb no íntron 3, em pacientes com neoplasia gástrica no Hospital Universitário João de Barros Barreto e em indivíduos controles da população miscigenada de Belém entre 01 de julho de 2009 a 01 de março de 2011. Foram coletadas 99 amostras de sangue de pacientes com câncer gástrico e 225 de indivíduos controles. O DNA foi extraído segundo o método descrito por Sambrook et al. (1989). Foi utilizada a técnica da Reação em Cadeia de Polimerase para a genotipagem das amostras. Os resultados obtidos indicam associação do câncer gástrico com o sexo masculino (67,7%) e idade (média de 59,3 ± 1,2 anos); predominância do subtipo difuso de Laurén e doença em estágio avançado (70,7% apresentaram TNM III/IV). Ficou evidenciada uma associação significativa de susceptibilidade ao câncer gástrico para os genótipos INS/DEL (p=0,007, OR=3,990) e INS/INS + INS/DEL (p=0,007, OR=3,554), além de antecipação aproximada de 15% no tempo de aparecimento da doença nos portadores do genótipo INS (p=0,002, OR= 1,15). O genótipo DEL/DEL apresentou-se relacionado à proteção ao desenvolvimento do tumor gástrico (p=0,020, OR=0,343).
