It is of great importance for the health of women, their families and society. However, in the midst of the SARS­CoV­2 virus pandemic scenario, proper practices are needed for the correct management of the milk offered to the baby, in order to ensure safety during breastfeeding and to prolong breastfeeding, since the prevalence rates of breastfeeding in Brazil are still much lower than recommended. Objective: To identify the importance of exclusive breastfeeding and the recommendations and challenges in the COVID­19 pandemic context. Methods: Integrative, explanatory bibliographic review study, with a qualitative approach to data from the Virtual Health Library (BVS)/BIREME, with the aid of the Latin American and Caribbean Literature in Health Sciences (LILACS), Scientific databases Electronic Library Online (SciELO) and Online Medical Literature Search and Analysis System (MEDLINE). Results: 59 articles were found and after selection by inclusion and exclusion criteria, 16 articles composed this review. For the analysis, tables were assembled with the main information from the descriptors: "Exclusive Breastfeeding", 06 (six) articles; "Components of the breast milk and the importance for the baby's health", 03 (three) articles; "Importance of breastfeeding for the mother", 02 (two) articles and "Advantages of breastfeeding and the COVID­19 pandemic" 05 (five) articles. Conclusion: With all the evidence of the importance of breastfeeding, it is necessary to increase the dissemination of information, aiming at increasing breastfeeding rates in Brazil, and providing more security for mothers when breastfeeding, since, in the pandemic context of COVID ­19, many uncertainties have arisen. However, it is indicated to continue the practice of breastfeeding based on biosecurity of materno Milk outweigh the benefits arising from infection by the new virus. Introdução: O leite materno é o alimento natural mais completo e essencial para o lactente, pois, contém nutrientes benéficos para atender as necessidades metabólicas do recém­nascido em seus primeiros meses de vida. É de grande importância para a saúde da mulher, sua família e para sociedade. No entanto, em meio ao cenário pandêmico do coronavírus são necessárias práticas adequadas para o manejo correto do leite ofertado ao bebê, com intuito de garantir a segurança durante a mamada e prolongar o aleitamento, visto que, os índices de prevalência do aleitamento no Brasil, ainda estão bastante inferiores ao recomendado. Objetivo: Identificar a importância do aleitamento materno exclusivo e as recomendações e desafios no contexto pandêmico da COVID­19. Métodos: Estudo de revisão bibliográfica integrativa, explanatória, com abordagem qualitativa dos dados a partir da Biblioteca Virtual em Saúde (BVS)/ BIREME, com o auxílio das bases de dados Literatura Latino­Americana e do Caribe em Ciências da Saúde (LILACS), Scientific Electronic Library Online (SciELO) e Sistema Online de Busca e Análise de Literatura Médica (MEDLINE). Resultados: Foram encontrados 59 artigos e após seleção pelos critérios de inclusão e exclusão, 16 artigos compuseram essa revisão. Para análise dos mesmos, realizou­se a montagem dos quadros com as principais informações a partir dos seguintes descritores: “Aleitamento Materno Exclusivo”, 06 (seis) artigos; “Componentes do leite materno e a importância para a saúde do bebê”, 03 (três) artigos 02 (dois) para “Importância da amamentação para a mãe”, 02 (dois) artigos e “Vantagens do aleitamento materno e pandemia COVID­ 19” com 05 (cinco) artigos. Conclusão: Mesmo com todas as evidências sobre a importância da amamentação é necessário aumentar a divulgação de informações, visando crescimento dos índices de aleitamento materno no Brasil, e proporcionar mais segurança para as mães na hora de amamentar, visto que, no contexto pandêmico da COVID­19, surgiram muitas incertezas.
