Blood transfusion, particularly through its components and derivatives, often plays a key role
in reestablishing the patients' physical integrity. Hemotherapy has provided support for several
treatments, such as transplants, surgeries and chemotherapy. The objective of this study is to
outline the profile of requests for blood components in an urgency and emergency hospital the
one from the north of Brazil, through which it traces an epidemiological profile of patients
submitted to transfusion therapy evaluating the indices of conformity of the adequations of the
indications and doses of blood components according to the Ministry of Health manual. In
addition, the main causes of blood transfusion requests and the associated care modality were
observed. The data for the research were collected from a form prepared by the authors with
information on the blood packet request forms of the transfusion agency. A total of 715 requests
were analyzed from 538 patients, 58% male and 42% female. The most requested blood group
was red blood cell concentrate, followed by platelet concentrate and fresh frozen plasma. Most
of the cases were of relative urgency and the minority corresponded to emergencies. The causes
that led to the requests for blood transfusion were diverse, highlighting hematological and
infectious as the most prevalente. Regarding the adequacy of the requests, the red blood cell
concentrate was mostly in compliance with the Ministry of Health manual, while the platelet
concentrate and fresh frozen plasma showed a large part of their indications and could expose
the patients to risks unnecessary. A high level of nonconformities was observed in relation to
the doses of blood components, mainly for the platelet concentrate and fresh frozen plasma.
Therefore, there is a clear need to implement measures aimed at reducing the indices of
nonconformities in the requests for blood components and their doses, taking into consideration
the current protocol, in order to reduce damages and life risks to patients. |
