The Necrotizing Enterocolitis (NEC) is a clinical-pathological syndrome which progresses with gastrointestinal signs and symptoms, resulting from progressive inflammation and necrosis of the intestinal wall. NEC has variable evolution, which currently makes a widely trusted biomarker unavailable or other clinical test to determine an individual patient's risk of NEC. The current standard is to closely observe all preterm infants for clinical signs of NEC. It is one of the most common surgical emergencies in neonates and accounts for 1-3% of neonatal admissions to intensive care units being 90 to 95% of these are premature. Recent findings based on a better understanding of the physiopathogenesis of NEC have the potential to serve as a basis for new preventive and treatment strategies. The NEC is one of the main factors of morbimortality in preterm infants, therefore, the knowledge about the best management of NEC is a major field of research nowadays. With the study, the objective was to carry out a bibliographical survey through an integrative review on the management of necrotizing enterocolitis in premature neonates. The study is an integrative literature review based on the analysis of articles published in scientific journals in order to explore the management of necrotizing enterocolitis in preterm infants. The electronic databases PubMed, SciELO, LILACS, Cochrane Library, Web Of Science and ScienceDirect were consulted retrospectively in the last 5 years 2018- 2022 (period chosen arbitrarily), using the following health sciences descriptors: necrotizing enterocolitis, combined with prematurity, with no language restriction. 85 articles were included in this study. Therefore, breastfeeding in the prevention of NEC and its early reintroduction in the treatment of the disease stands out, as well as the use of probiotics in a preventive way. Laparotomy remains the main surgical treatment in cases of severe NEC, and there is no recommended antibiotic regimen for the treatment. The NEC is a challenge for neonatology and there is still no ideal standard management for those affected by the disease, which increases the morbimortality. |
