It is an empirical-analytical, bibliographic and exploratory research. The items that make up the instrument were prepared based on theoretical formulations and empirical results on critical and creative thinking due to the Research and Investigations Group in Mathematics Education, linked to the University of Brasília. The elaboration of the items occurred under the principles of the Classic Test Theory. In addition, three examples of how the teacher can adapt issues of the National High School Exam (ENEM) are presented to stimulate the creativity and criticality in mathematics of high school students in this perspective. The built instrument can serve as a starting point for further work with a similar objective, to the development of learning expectations about critical and creative thinking in mathematics to guide teachers and students in the construction of such skills or as a state of the art of the Pi Group research the theme. O objetivo da dissertação é propor um instrumento para levantamento de informações sobre percepções de professores de matemática sobre Criatividade e Pensamento Crítico em Matemática. Trata-se de uma pesquisa empírico-analítica, bibliográfica e de natureza exploratória. Os itens que compõem o instrumento foram elaborados com base nas formulações teóricas e resultados empíricos sobre pensamento crítico e criativo devidos ao Grupo de Pesquisas e Investigações em Educação Matemática, vinculado à Universidade de Brasília. A elaboração dos itens ocorreu sob os princípios da Teoria Clássica dos Testes. Complementarmente, são apresentados três exemplos de como o professor pode adequar questões do Exame Nacional do Ensino Médio (ENEM) para estimular a criatividade e a criticidade em matemática de alunos do ensino médio nesta perspectiva.
