This study aimed to identify the conceptions of Community Health Agents in the municipality of Miracema do Tocantins (TO) regarding Harm Reduction strategies and the logic of abstinence. Additionally, it sought to understand how these conceptions influence the practices of Community Health Agents (ACS) directed towards alcohol and other drug users. To achieve this, semi-structured interviews were conducted with two Community Health Agents who have been working in the municipality for twenty-four years. This study was based on a qualitative, exploratory, and field research approach, using Bardin's content analysis technique. From the data analysis, three categories were identified: 1) ACS involvement with alcohol and other drug users: challenges and possibilities; 2) ACS training in mental health for approaching alcohol and other drug users; 3) Depositions regarding harm reduction and abstinence: drug and user stigmatization. Drawing on the theoretical framework of Lacanian psychoanalysis, the aim was to understand how ACS, as professionals in Primary Care and key "bridges" between the Health Care Network (RAS) and the community, can work with alcohol and other drug users. The study also explored how Psychoanalysis and Harm Reduction can contribute to their practice. The results highlight that the Agents' perception is influenced by a psychiatric and stigmatizing logic, stemming from one of the main challenges faced by professionals: insufficient and deficient training in the field of alcohol and drugs. The importance of the role of Community Health Agents in the field of alcohol and other drugs was evident. Despite the challenges, the practice of ACSs can be seen as an opportunity to construct strategies based on their privileged position in the community's daily life.
