As time went by, more conservative methods were studied without risks to leave residual illness. Therefore it follows that new methods have been developed for staging and treatment of the axilla as well. Axillary lymph node dissection comes with a substantial morbidity (lymphedema, sensory loss and pain). Although these complaints are usually not severe, the incidence is high. In this context, the search of the sentinel lymph node (SLN), brought new perspectives in the axillary boarding, in this type of cancer. The SLN is defined as the node that receives afferent lymphatic drainage directly from a primary tumor. According to searches in this area, the axillary lymph node dissection should be accomplished only when the SLN is compromised with metastasis cell. Objectives: The present study evaluates the effectiveness of search of sentinel lymph node in the breast cancer. At that way, only those patients who have metastasis axillary will be submitted to axillary dissection, reducing the morbidity. Methods: It was included in this study 86 patients with breast cancer histologically confirmed, who were submitted to the lymphoscintigraphy to look for SLN, in the period of 2001 and 2005. Results: It was shown that axillary dissection was unnecessary in patients with negative SLN. Conclusion: The search of the LS presents high negative predictive value, which contributes to reduce the morbidity in patients with breast cancer. A Mastectomia radical com esvaziamento axilar completo foi, por muito tempo, o padrão-ouro no tratamento do câncer de mama. Com o decorrer dos estudos nesta área, procedimentos cada vez mais conservadores foram sendo utilizados, sem risco de deixar doença residual. Sendo assim, novos métodos foram sendo desenvolvidos para o estadiamento e tratamento da axila. A linfadenectomia axilar acarreta grande morbidade às pacientes como linfedema, dor crônica, parestesia entre outros, com elevadas taxas de incidência. Nesse contexto, a biópsia do linfonodo sentinela (LS) revolucionou a abordagem axilar das pacientes com câncer de mama. O LS é definido como o primeiro linfonodo a receber as células metastáticas provenientes do tumor primário. Com isso, passou-se a questionar o valor da linfadenectomia axilar e se, realmente, todas as pacientes seriam beneficiadas com este procedimento. Segundo os estudos, a linfadenectomia axilar só deve ser realizada se o LS estiver comprometido com células neoplásicas. Objetivos: O presente trabalho avalia a eficácia da pesquisa do linfonodo sentinela no câncer de mama. Identifica, ainda, aquelas pacientes que realmente necessitam submeter-se a linfadenectomia axilar, com a finalidade de reduzir a morbidade deste procedimento. Métodos: Foi realizada a coleta e análise de dados de 86 pacientes com diagnóstico de câncer de mama histologicamente confirmado, submetidas à linfocintilografia mamária para pesquisa do linfonodo sentinela, no período de 2001 a 2005. Resultados: A pesquisa mostra que nas pacientes que possuem diagnóstico confirmado de câncer de mama e presença de linfonodo sentinela negativo à biópsia, a linfadenectomia torna-se desnecessária.
