Resumo:
Diabetes mellitus (DM) stands out as a major public health problem, being regarded as a disease of epidemic proportions throughout the world. It is estimated that in Brazil there are currently about 8 million individuals with DM, who suffer the debilitating effect exerted on the extremities, mainly in the feet, leading to the development of what is considered as a "syndrome of diabetic foot", one of mutilantes more complications of chronic illness before the socio-economic observed throughout the world, which can be solved or eased through programmes to comprehensive health care for the feet, from the integrated network with the basic units of greater complexity. The purpose of this study is to propose a model of multidisciplinary assistance for care of diabetic patients with disorders of the lower end in the municipality of Belém-Pa. The methodology as well as several of the parameters used, based on the “Programa de Atenção ao Pé Diabético”, project implemented by the SMS-RJ to develop a programme of assistance for hierarchical multidisciplinary prevention and treatment of disorders of the lower end of patients diabetics, using the networks basic, secondary units and hospitals of reference of the Single Health System of the city of Belém-Pa. The literature shows the effectiveness of programmes for monitoring the multidisciplinary diabetic individuals with lower end of disorder, which improve the quality of life of patients, reduce the occurrence of complications and can optimize the use of resources. Hence the importance of a proposal for assistance of diabetic patients with neuro-vascular complications in the feet that seek the care of the patient in a hierarchical, aiming to reduce the morbidity, mortality complications of the lower limbs and socio-economic cost of disease.
