The patients were divided in two groups, where the first one with 30 patients who had normal glycemic values (NG group) and the second one with 10 patients who had insulin resistance (RI group). The groups’ follow-up was made on the third and sixth months. In both groups the BMI decreased 30% among with glycemia, insulin, cholesterol and its fractions and triglycerides values. The insulin resistance was significantly reduced on the third month on RI group compared with the NG group, but the abdominal circumference decrease more on this group after 3 months. The metabolic syndrome decreased 40% on RI group and 6,6% on NG group. Conclusion: Bariatric surgery was benefical in both groups. All parameters used on this study (cholesterol its fractions, triglycerides, glycemia, insulin and the metabolic syndrome association) had a significant decrease on the follow-up 3 and 6 months postoperative in both groups. Fez-se uma analise prospectiva de 40 pacientes submetidos à cirurgia bariátrica entre 2007 e 2008, para estudar os efeitos da cirurgia em relação à resistência à insulina, os valores de glicemia, insulina, colesterol e suas frações, triglicerídeos e a associação com a síndrome metabólica. Os pacientes foram divididos em dois grupos, sendo o primeiro com 30 pacientes que tinham valores glicêmicos normais (grupo NG) e o segundo com 10 pacientes que tinham resistência à insulina (grupo RI). O acompanhamento destes grupos foi feito no terceiro e no sexto mês de pós-operatório. Em ambos os grupos, o IMC caiu 30% em seis meses juntamente com os valores de glicemia, insulina, colesterol e triglicerídeos. Houve queda significativa da resistência insulínica no grupo RI no terceiro mês em comparação com o grupo NG, porém este apresentou queda maior na circunferência abdominal após três meses. A síndrome metabólica caiu em 40% no grupo RI e 6,6% no grupo NG. Conclusão: a cirurgia bariátrica foi benéfica para os dois grupos.
