In this context, the surgical approach is one of the therapies of choice. Among the various surgical techniques, the Roux-en-Y gastric bypass is the most performed worldwide, corresponding to about 45% of total procedures, and is also the most costly from a financial point of view. In Brazil, the medical literature still lacks studies that analyze and compare the costs and mortality of these surgical procedures. Based on these data, the present research aims to study the financial data and mortality rates involving total and vertical gastrectomy surgeries notified and registered nationally in the Unified Health System. Cases of patients undergoing medical procedures categorized as digestive tract surgery were analyzed, with 2,118 hospital admissions for laparoscopic bariatric surgery in Brazil between 2017 and 2020 recorded. The Northeast and Southeast regions have the most expenses related to procedures, corresponding to 75.56% of all expenses. The North, on the other hand, accounts for just 1% of all expenditures. The total expense for the treatment of complications after bariatric surgery was R$1,415,240.00 reais in the years analyzed. The South region was responsible for 83.94% of expenditures, the Northeast for 3.97%. The total number of deaths, 78.57% occurred in the South region, followed by the Southeast region with 16.67%. The highest mortality rate occurred in the State of São Paulo with 10.34. It is concluded that there is a greater concentration of the absolute number of procedures in the Northeast, and there are more hospitalizations for Videolaparoscopy than Sleeve Gastrectomy, and as for the cost, it was noted that the Northeast and Southeast regions have the most expenses, whereas the mortality rate is led by the South and Southeast regions. A obesidade é considerada uma doença crônica não transmissível, consistindo em um dos principais agravos de saúde pública da atualidade, com sua prevalência aumentando rápida e expressivamente nas últimas décadas, com uma série de repercussões deletérias com o aumento da morbimortalidade. Nesse contexto, a abordagem cirúrgica é uma das terapêuticas de escolha. Dentre as diversas técnicas cirúrgicas, a derivação gástrica em Y de Roux é a mais realizada mundialmente, correspondendo a cerca de 45% dos procedimentos totais, sendo também a mais custosa do ponto de vista financeiro. Sendo que no Brasil, a literatura médica ainda carece de estudos que analisem e comparem os custos e a mortalidade desses procedimentos cirúrgicos. A partir destes dados a presente pesquisa almeja estudar os dados financeiros e as taxas de mortalidade envolvendo as cirurgias de gastrectomia total e vertical notificadas e registradas nacionalmente no Sistema Único de Saúde. Foram analisados os casos de pacientes submetidos a procedimentos médicos categorizados como cirurgia do aparelho digestivo, registradas 2.118 internações hospitalares para a realização de cirurgia bariátrica por videolaparoscopia no Brasil entre 2017 à 2020. Foram analisadas 1.005 gastrectomias verticais em Manga entre o período estudado. A região Nordeste e a Sudeste são as que mais possuem despesas relacionadas aos procedimentos correspondendo a 75,56% de todos os gastos. O Norte, por outro lado, equivale apenas a 1 % de todos os gastos. O gasto total para o tratamento das complicações pós cirurgia bariátrica foi de R$1.415.240,00 reais nos anos analisados. A região Sul foi responsável por 83.94% dos gastos, o Nordeste por 3,97%. O número total de óbitos, 78,57% ocorreram na região Sul, seguido da região Sudeste com 16,67%. A maior taxa de mortalidade ocorreu no Estado de São Paulo com 10.34.
