The facial appearance in these patients is compromised, being one of the factors that motivates them to seek treatment for this type of malocclusion. The improvement in the facial aesthetics after the treatment contributes to raise the patient's self-esteem, as well as the psychosocial and functional improvement. Class III is considered challenging for the orthodontist, particularly when there is skeletal involvement, and joint planning with a maxillofacial surgeon is required to meet the patient's functional and aesthetic needs. The objective of this study is to describe a clinical orthodontic surgical case of class III malocclusion from the planning to the surgical procedure. The clinical characteristics presented were class III malocclusion due to maxillary retrognathism and associated mandibular protrusion. Great dental compensation with inclinations and positioning in their respective altered bone bases. Speech pathology and impaired swallowing, as well as compromised aesthetics. The proposed treatment was the surgical orthodontic with the preparation prior to surgery and refinement of the post-surgery case. As más oclusões de Classe III caracterizam-se por um posicionamento anterior da mandíbula em relação à maxila, considerando que a discrepância pode ser causada por o déficit de crescimento maxilar, prognatismo mandibular excessiva ou a combinação de ambos. O aspecto facial nesses pacientes fica comprometido, sendo esse um dos fatores que os motiva a procurar tratamento para esse tipo de má oclusão. A melhora na estética facial após o tratamento contribui para elevar a autoestima do paciente, assim como a melhoria psicossocial e funcional. A Classe III é considerada desafiadora para o ortodontista, particularmente quando há envolvimento esquelético, sendo necessário um planejamento em conjunto com um cirurgião bucomaxilofacial para atender às necessidades funcionais e estéticas do paciente. O objetivo desse trabalho é descrever um caso clínico ortodôntico cirúrgico de má oclusão de classe III desde o planejamento até o procedimento cirúrgico. As características clínicas apresentadas foram má oclusão de classe III por retrognatismo maxilar e protrusão mandibular associadas. Grande compensação dentária com inclinações e posicionamento em suas respectivas bases ósseas alteradas. Aspectos fonoaudiológicos e deglutição prejudicada, assim como estética comprometida.
